USSF Assessor Forms and Policies
The following are useful field and office materials for assessors. These publications provide procedures and applications as defined by the U.S. Soccer Federation and are available with other content, not listed here, through www.ussoccer.com in electronic format.
Several of these forms may be utilized as field observation sheets to assist in note taking that would apply to the assessor’s final online written report to be submitted at cnra.gameofficials.net – CNRA: California North Referee Administration (Group 1271).
