  • All participants of the CNRA Referee Academy Field Sessions are required to fill out, sign and submit the following document to your session administrator.
  • Those under the age of 18-years-old must have a parent and/or guardian also sign the document.
  • Participants only need to submit this form once during the current season.
  • Forms shall be provided at the field should you not have one.
